The document below is our volunteer membership waiver.  Remember LEPA operates a performing arts company that does everything from the door to the floor; which includes writing, singing, dancing, acting, choreography, directing, producing, graphic arts, creative artistry, stage crew, set-design, lighting, administration, customer service, sales, marketing, advertisement, etc...  There is always something for you to do.

Download the form and print page 2.  Fill it out for each individual performer and submit it to LEPA.ARTS@GMAIL.COM.
